Tracking what page a form was submitted on

I have a rather large database that I'd like people to suggest links that are related to a particular entry. (It is the languages of the world, about 7,000 posts) and I'd like a way to tie the submitted data to the page in which the [wplinkdir page="Add URL"] code was placed. Once entries are approved I'd like them to appear under the posts where they were created. http://www.ethnolinks.com/blog/?p=88.

Tracking on what page your link was posted on is an easy task!!
Just open that forum and then check in which thread you posted!!Thereafter you can search for the topic!!
